Medical Cannabis Patient Program (MCPP)

If you are seeing this instruction manual that means you have been approved by your physician and you are one step closer to receiving your medical cannabis card. Congratulation!


Please read the instructions below as we provide valuable tips on how to best complete your application.

Step 1: Qualifying Requirements

To qualify for a patient registry identification card, a qualifying patient must:

  • Be a resident of the State of Illinois at the time of application and remain a resident during participation in the program;

  • Have a qualifying debilitating medical condition;

  • Have a signed physician certification (unless you are a veteran receiving medical care at a VA facility);

  • Be at least 18 years of age;

  • Not hold a school bus permit or Commercial Driver’s License (CDL); and

  • Not be an active duty law enforcement officer, correctional officer, correctional probation officer, or firefighter.

  • Make sure your physician completes the Physician Written Certification Form (pdf document) that you will upload with your on-line application or include the original document with your paper application - we send you the Physician Written Certification form as a Pdf in your approval email.

Step 2: Submit the Application Using Pointers Below

  • Navigate to the online application:

  • If you have not created your account previously, then Click on “Register” and provide the requested information.

  • Tip: once you fill-out the requested information you will receive a confirmation email. You will need to click on the link on the confirmation email in-order to proceed with the application.



  • Log back into your online portal ->Scroll Down -> Click on “Create a Patient Registration”.


  • Start the application that applies to you. For example, if you are a new patient with no terminal illness Click on Start for a Qualifying Patient Application and follow the application wizard.


Tip: For Step # 25. Please select your certifying health care professional from the authorized Illinois list. You will need to check your physicians name in your Physician Written Certification Form – provided to you in your approval email (see example below). LEAVE CITY FIELD BLANK.


Tip: For Step # 33. For proof of residency, attach two of the following items separately; Documents must match the name and address on your application. You will need to make sure that the address on both documents you provide matches.

Tip: For Step # 36. Photograph for Registry Card, you need to make sure that your photograph is at least 2x2 and is at least 600 x 600 pixels, but no greater than 1,200 x 1,200 pixels in dimension. You can use apps available on your phone to take your photograph (i.e. search for Passport Photo app in your mobile app store.)

Tip: For Step # 38. Please upload a .pdf scanned copy of your Health Care Professional Certification from your doctor – use the file provided to you in your approval email.

  • Carefully review your application before submission and make your payment to the state.

Tip: If the qualifying patient submits an application for MCPP on-line before 6 PM CST, they can download their provisional (temporary) registration card the following day, after 8:30 AM.  It is recommended the patient call their dispensary to verify their provisional registration card is active before driving to the dispensary location dispensary.

Step 3: Please follow the instructions below to download and print your provisional medical cannabis registration:



Tip: The provisional registration card is valid for up to 90 days. We understand that it make take 90-120 days to receive this. Please make sure to call your selected dispensary before your first visit to have them check you are in the state bio track system, we have been hearing there may be a day lag time.
