What Are The Requirements To Apply For A Medical Card In Missouri?

Be 18 years of age or older You must be able to provide proof of your Missouri residency You must have a diagnosis that qualifies in Missouri You must submit a signed Physician Certification with your MMP application – and that is where we come in! We will provide the Physician Certification and then you can apply to get your card.

What Are The Qualifying Conditions To Get A Medical Card in Oklahoma

A terminal illness Cancer Epilepsy Glaucoma Human immunodeficiency virus or acquired immune deficiency syndrome Intractable migraines unresponsive to other treatment A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with: Multiple sclerosis Parkinson’s disease Seizures Tourette’s syndrome Debilitating psychiatric disorders, including, but not limited to: Post-traumatic stress disorder, if diagnosed by a state licensed psychiatrist A chronic medical condition that is normally treated with prescription medications that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to: Agitation of Alzheimer’s disease Amyotrophic Lateral Sclerosis (Lou Gehrig’s Disease) Autism Cachexia (also known as Wasting Syndrome) Crohn’s disease Hepatitis C Huntington’s disease Inflammatory bowel disease Neuropathies Sickle cell anemia
