Requirements To Apply For A Medical Card In IllinoisBe 18 years of age or older You must be able to provide proof of your Illinois residency You cannot have a Commercial Driver’s License (CDL) You must have a diagnosis that qualifies in Illinois You must submit a signed Physician Certification with your IDPH application – and that is where we come in!
